Full Remaster
This is the recommended way, using grml-live. This process is a full rebuild, building a completely new squashfs. It gives you full control about the result, including removing and adding packages, configuration etc.
To remaster, you need a Debian testing/sid installation or a current grml itself. Running the installation system in a VM is fine. The remastering process doesn't anything weird outside the build directory. It's possible to use so called (root) persistency on Grml.
This means you can store (all) your settings and reuse them on reboot, without having to remaster/re-generate the ISO by storing all the changes on an external media (like a USB device).

Preface
The Grml rescue system supports booting the Grml ISO from a hard disk. Most computers today have the capability to boot over the network. This network boot capability is often named PXE (Preboot eXecution Environment). This can be very handy to boot into Grml without burning CDs, writing USB sticks, etc.

Grml officially supports booting from usb-stick with all grml flavours. First of all make sure your system supports booting via usb/firewire. 'apt-get install foo' seems to freeze after a while
It seems you are running out of RAM - see 'free' while running 'apt-get...'. UnionFS and the debian packetmanagement require some more RAM for the ramdisk.
